**RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT**

One Matters Inc. and it’s representing officers, directors, employees, agents, volunteers, contractors, sponsors, and all affiliated entities)

In consideration of being permitted to participate in the above-referenced charity boxing competition and related activities, including but not limited to training, weigh-ins, bouts, sparring, warm-ups, cool-downs, and use of the facility and equipment, I, the undersigned Participant (or Parent/Legal Guardian if the Participant is a minor), do hereby agree as follows:

1. Acknowledgment and Assumption of Risk I understand and acknowledge that **boxing and combat sports are inherently dangerous activities** that involve significant risks of serious bodily injury, permanent disability, paralysis, death, and property damage. These risks include, but are not limited to: - Strikes, impacts, and trauma from punches, falls, or collisions with opponents, ropes, or the ring/floor. - Concussions, head injuries, broken bones, sprains, strains, cuts, bruises, eye injuries, dental injuries, or internal injuries. - Exacerbation of pre-existing medical conditions (e.g., heart issues, neurological conditions). - Risks from other participants (including competitors), equipment failure, inadequate supervision, facility conditions, or negligence of others. - Travel to/from the Event and any related activities.

I voluntarily assume **all risks**, known and unknown, even if arising from the negligence of the Organizer or Releasees (defined below), and I agree to accept full personal responsibility for any injury, loss, or damage I may suffer.

2. Release and Waiver of Liability I, on behalf of myself, my heirs, executors, administrators, assigns, and next of kin, hereby **release, waive, discharge, and covenant not to sue** the Organizer, the facility/venue owners/operators, all sponsors, promoters, officials, referees, medical staff, volunteers, other participants (including competitors), and their respective officers, directors, employees, agents, insurers, and successors (collectively, the "Releasees") from **any and all liability** for any claims, demands, losses, damages, injuries (including death), causes of action, or expenses (including attorneys' fees) arising out of or related to my participation in the Event.

This release includes claims based on **ordinary negligence** (to the fullest extent permitted by law), but does **not** release claims for gross negligence, willful misconduct, or intentional acts.

3. Indemnification and Hold Harmless I agree to **indemnify and hold harmless** the Releasees from any and all claims, liabilities, damages, costs, or expenses (including attorneys' fees) brought by me, my family, representatives, or any third party as a result of my participation in the Event, including claims arising from my actions or inactions, or from the actions of other participants/competitors.

4. Medical Fitness and Emergency Treatment I certify that I am physically fit and have sufficiently trained for the Event. I have no known medical conditions that would prevent safe participation, or I have disclosed them in writing. I authorize the Organizer and medical personnel to seek emergency medical treatment for me if needed, and I agree to be responsible for any related costs. I understand that the Organizer does not provide medical insurance, and I am responsible for my own coverage.

### 5. Rules Compliance and Conduct I agree to abide by all Event rules, safety protocols, and instructions from officials. I understand that failure to do so may result in disqualification or removal from the Event without refund or recourse.

### 6. Media and Likeness Release I grant the Organizer and its designees the irrevocable right to use my name, image, likeness, voice, and statements in any media (photos, video, audio, etc.) for promotional, charitable, or archival purposes without compensation.

### 7. Severability and Governing Law If any provision of this Agreement is held invalid, the remainder shall continue in full force. This Agreement shall be governed by the laws of the State/Commonwealth of [Insert Your Jurisdiction], without regard to conflict of laws principles. Any disputes shall be resolved exclusively in the courts of [Insert County/Jurisdiction].

### 8. Voluntary Execution I have read this Agreement carefully, fully understand its terms, and sign it **freely and voluntarily** without any inducement. I intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law.
